What is MyMedicare?

MyMedicare is a voluntary patient registration scheme from the Australian Government.

 

It is designed to strengthen the relationship you have with our practice, your preferred GP, and our primary care team.

By registering, you help us to better understand your health needs and provide you with more connected, high-quality care tailored to you.

 

Evidence shows that seeing the same GP and healthcare team regularly leads to better health outcomes.​

Registering with our practice in MyMedicare provides several benefits including:

​

  • Greater Continuity of Care: A formal relationship with our practice and your preferred GP ensures you receive consistent and coordinated care.

​​

  • Improved Health Outcomes: Seeing the same healthcare team regularly can lead to better management of chronic conditions and overall health.

CARE PLANS & REVIEWS

Chronic Condition Management

​

GP Management Plans and Team Care Arrangements have been replaced with a streamlined GP Chronic Condition Management Plan (GPCCMP).  You might know this as your "Care Plan" and "Care Plan Review" appointments. 

​

Please note: If you are registered with MyMedicare, you must access your chronic condition management plan through the practice where you are registered. This is a Medicare requirement to support continuity of care.

​

This means that if you are registered with our practice through MyMedicare, we can provide your chronic condition management plans and reviews. However, if you are registered with another practice (or not registered at all), we are unable to provide these services as they will not qualify for Medicare funding.

​

We encourage all patients with chronic conditions to register with our practice through MyMedicare so we can continue to provide comprehensive chronic disease management and allied health referrals.

WHO CAN REGISTER

Who can Register?

You are eligible to register if you:

  • Have a Medicare card or Department of Veterans' Affairs (DVA) Veteran card

  • Have had two face-to-face visits with our practice within the last 24 months

​​

(Note: Patients in residential aged care homes are exempt from the face-to-face visit requirement)

HOW DO I REGISTER

How do I register?

Registering is simple and you can choose the method that suits you best:

Option 1: Register Online

​You can register using your Medicare Online Account through your myGov account or the Express Plus Medicare mobile app:

  1. Sign in to your myGov account and link Medicare if you haven't already

  2. Select Medicare, then select "View MyMedicare"

  3. Follow the prompts to search for our practice: The Family & Diabetic GP Clinic

  4. Select your preferred GP

  5. Complete the registration

Register for MyMedicare via Services Australia

​

Option 2: Register In-Person at Our Practice

The next time you visit us, simply ask our reception team about MyMedicare registration. We can provide you with a MyMedicare Registration Form to sign, giving your consent for us to complete the registration process for you.

CHANGING REGISTRATION

Can I change my Registration?

​

Yes! You can change or withdraw your registration at any time, including:-

​

  • Change your preferred GP within our practice at any time

​​

  • Change to a different practice if you meet the eligibility requirements

​​

  • Withdraw from MyMedicare at any time through your Medicare Online Account

​​

When you register with a new practice, your previous registration is automatically cancelled.
